All the forms that you need to complete your registration are listed to the left. Please complete and bring with you to registration. Registration will be both Friday night and Saturday morning for your convenience.

AIM Team Roster - FILL IN THE NAMES OF PLAYERS YOU BRING AND THEIR JERSEY NUMBERS - 2 copies please
Medical Release - BRING YOUR CLUB MEDICAL RELEASE FORMS FOR US TO VERIFY AND GIVE BACK TO YOU or FILL OUT FORM ON THE LEFT
Guest Player Medical Release - IF YOU BRING A GUEST FROM ANOTHER CLUB, THEY MUST FILL OUT THIS FORM
Official State Certified Roster - A COPY OF YOUR OFFICIAL ROSTER TO BE KEPT BY US
Player Passes - YOU MUST PRODUCE A PLAYER PASS FOR EACH PLAYER ON THE AIM ROSTER AND HAVE AT EVERY GAME
Permission to Travel Form - IF FROM OUTSIDE NY STATE WEST DISTRICT
