All the forms that you need to complete your registration are listed to the left. They can either be downloaded and opened on your PC or you can open them using your browser. To download right click on the link and click save target as, a dialog box will pop up and you can save to any folder on your computer. Please complete and bring with you to the registration center at the tournament. Registration will be both Friday night and Saturday morning.
Team Checklist - A listing of the tasks that each team must complete for registration to help you keep organized.
The forms that are needed are as follows:
Team Roster - this must be completed with the information on each of the players that will be used in the tournament. This will be checked with the players cards and validated at registration. This is then stamped and used as your roster for the tournament. You will still need to have your passes ready for the referees to spot check at the game sites.
Team Medical/Liability Release - Each player, if over 18, or parent must sign this one form. Each Coach certifies that they have medical release forms for each regularly rostered player and has copies with them.
Guest Player Medical Release - this form is used only when a guest player is being used in the tournament. The guest player must have a medical release form completed. The guest player will also sign the Team Medical/Liability Release form.
Certified Roster - copy of your certified team roster validated by your local district office.
Permission to Travel - if you are registered with any district other than New York State West you will need to have a permission to travel form for registration.
TShirt Preorder Form - this would be used if your team would like to preorder tournament tshirts. They would be available at registration. This form must be mailed to the Tournament Director by May 10, 2008 to be available by registration. There will be a small number of tournament t-shirts available at the event. Your best option is to pre-order.
Please complete all the forms and bring them to registration. Please bring everything to registration. You do not need to mail anything in before registration.
