Instructions to Complete the Risk Management Process for the Amherst Soccer Association

 

All Amherst Soccer Association house league coaches must have a valid pass.

 

Other house league volunteers (assistant coaches and team managers) do not need to have a pass, but should be registered by our Risk Management Coordinator with the New York State West Youth Soccer Association (NYSWYSA) in order to be covered by our insurance.

 

NYSWYSA passes are valid for 2 years. To determine if your pass is valid, check the expiration date on your pass, or email our Risk Management Coordinator, at riskmanager@amherstsoccer.com, and she will check the status of your pass for you.

 

  1. If you know your pass has expired, or if you are requesting a pass for the first time, please email riskmanager@amherstsoccer.com with your full name, address, phone number(s), birth date and preferred email address. Your information will then be entered, and your request for a background check and a pass will be sent to NYSWYSA.

 

  1. Once NYSWYSA has received your request, they will send you an email requesting additional personal information. NYSWYSA will also have you download a hard copy of their disclosure form, which you will fill out completely and legibly, sign, and mail back directly to them. The last 3 lines of the form under “Passes shall be returned to:” must be filled in with the name and address of our Risk Management Coordinator, which you will receive in an email directly from her. Any form that is missing information, or that is not legible, will be promptly returned. You do not need to send the $6 fee. NYSWYSA will bill the Amherst Soccer Association directly.

 

  1. When the hard copy of your disclosure form is received in the NYSWYSA office (with the necessary signature giving them permission to do the background search), your background search will be executed.

 

  1.  When your pass has been approved, NYSWYSA will return your pass to our Risk Management Coordinator, who will then mail the pass to you. Please allow at least three weeks for the requests to be processed completely.

 

  1. Since house league coaches are not required to show their pass prior to games, it is not required, but it is recommended, that you make your pass valid by attaching a recent photograph, signing the pass using all the names and initials printed on the front of the pass, and having the pass laminated.

 

 

Please Note: the cost for replacing a current pass that is lost is $25. To obtain a replacement pass you do not need to fill out another NYSWYSA disclosure form; however, you must send a letter to the NYSWYSA State Office requesting the replacement pass along with the proper payment.
